This dog/puppy contract is entered into on ____________________________2010 by and between, Fairy French (seller/breeder), and _______________________________________________(purchaser).
1) The seller/breeder agrees to sell and the purchaser agrees to buy the following described dog/puppy:

2) The price of this dog/puppy in

4) This dog/puppy is being sold as a PET only. The purchaser agrees to neuter/spay this dog/puppy by 6 months of age. Under no circumstances shall this pet quality dog/puppy be used for breeding. After dog/puppy has been neutered/spayed, purchaser agrees to send to seller/breeder a veterinarian certificate as verification, and seller/breeder will then forward dog/puppy Signed Registration Certificate.
5) The dog/puppy is guaranteed to be in good physical condition at the time of delivery. It is recommended that dog/puppy be examined by your veterinarian within first 3 days. If the dog/puppy is found to have a genetic condition not detected prior to delivery, the seller/breeder agrees to take the dog/puppy back and replace with another dog/puppy of pet value when one is available.
Purchaser signature indicates full agreement and approval of all terms and conditions of this contract.
